Service Options Using Resources in a Community Environment (SOURCE)
SOURCE connects authorized long-term health services in a person’s home or community with primary care and case management. Every SOURCE client needs to fulfill the long-term facility level of care requirements and be eligible for full Medicaid. SOURCE offers home-and-community-based services to aged, frail, and physically challenged individuals under the Georgia aged and Disable (1915c) Medicaid Waiver. To ascertain eligibility and need, a thorough evaluation is conducted. Services Provided Below:
